Advance Directive

People of all ages should have an Advance Directive, to help ensure that their health care wishes are carried out in the event of incapacity. This form includes the ability to describe preferences for life-sustaining treatment and to designate a health care agent.

You do not need a lawyer to complete. If you do need assistance in completing the form, contact your primary care physician, your local hospital, or Utah Legal Services (any Utah resident age 60 or older can call 1-800-662-1772. between 9 am and 2 pm for assistance).
